Abstract
As a preliminary study, a literature survey has been conducted. The survey aims at researching current thinking on techniques useful for prevention and investigation of major fires and other accidents in industrial settings. The increasingly complex technologies used in major industries, as for instance chemical processing, mean that while day-to-day operations are without incident, there is also the potential for serious disasters when combinations of events defeat usual safeguards. 125 books or articles were selected for review. Most of the titles pertains to Human Factors related safety research. Key concepts are that disasters are caused well in advance, that the decisions are made by those in supervisory or management positions, that in making decision risks are unknown or ignored and that the decisions cause normal safety mechanisms to be inadequate. Throughout the literature reviewed direct or indirect reference was made to management's role in disaster prevention.
