- Dense black smoke, greatly intoxified by burning polyurethane seats, and...the 30-minute breathing apparatus, were the major problems hampering rescue and fire-fighting operations. Anticipating just such a major disaster, BART's Safety Department had received bids for four-hour breathing equipment...(but) could not accept delivery of the units, however, because they had not yet been approved by the National Institute of Occupational Safety and Health.
